Of course, you can remove the scapular if required for surgery, we must obey this. Some might say, "I will carry the scapular invisibly in my heart," because we are not pagans.

My salvation does not depend on the material scapular, it depends on my faith and, in this case, my trust in the intercession of Our Lady, symbolized by this piece of her vestment. It is not the material piece itself, but my trust that matters.

For example, if you are imprisoned and your scapular is taken from you by force, you can still be without the material scapular, but you are still carrying it invisibly in your heart. The same applies to the situation with surgery.
